Knees Ankles Back Shoulders Wrist None of the aboveSECTION FOUR - Training waiverPlease select that you understand and agree to the following: I hereby waive and release Ignite Fitness and Nutrition Ltd and it successors and assigns, from any and all claims, costs, liability and expense for any injury, loss or damage whether known, anticipated or unanticipated arising from voluntary participation. I understand that qualified health and fitness professionals lead exercise activities, these may be employees or independent contractors. I agree not to hold Ignite Fitness and Nutrition Ltd responsible for the actions or omissions of employees, independent instructors or other participants.SECTION FIVE - Potential risksPlease select that you understand and agree to the following: I recognise that exercise may involve strenuous physical activity including, but not limited to, muscle strength and endurance training, cardiovascular conditioning and training, and other various fitness activities. I understand that I am participating at my own risk regardless of pre-existing physical or mental conditions or ailments. It is my responsibility and the responsibility to inform the health or fitness professional supervising of any existing conditions, whether physical or mental, and challenges that may affect participation in exercise. I understand that any exercise or fitness activity involves risk including but not limited to; slips, trips, falls, injury, abnormal changes in blood pressure, fainting, a risk of heart attack, stroke, other serious disability or death. I am accepting such risks and volunteering to participate with full understanding of the dangers involved. In the event that medical attention is required, I consent to treatment by qualified first aiders on site in the first instance, and if further treatment is required that the minor can be transported to the nearest emergency centre, including by ambulance if necessary.SECTION SIX – Personal likenessWe want to protect your image, and your choice. Only tick the box if you're happy for us to use your image or likeness. I give Ignite Fitness and Nutrition consent to use my name and images on their websites, Facebook pages and advertising promotional material.SECTION SEVEN – Training Programme Terms and ConditionsGeneral terms - Pricing and Payment All memberships automatically renew every 4 weeks, but you have the option to cancel at any time. All training sessions must be used within the 4-week billing period and cannot be carried over to the following 4-week period. All prices are accurate at the time of quoting. We reserve the right to adjust fees, and if necessary, we will notify you 28 days in advance of any such changes. Memberships cannot be put on hold.Membership Cancellation To cancel a membership, a 28-day written notice is required with no exceptions.SECTION EIGHT – Personal Property Ignite Fitness and Nutrition Ltd is not responsible for any loss or damage of personal property.SECTION NINE – Use of the facilities Use of the facilities is subject to facility protocols. Staff instructions must be followed at all times.Acceptance of Terms and ConditionsBy completing this form, you acknowledge and agree to the terms and conditions outlined above.
